Problem

Charges were delayed or incomplete after busy evenings. Eligibility was sometimes verified at registration but not re-checked when coverage changed mid-week. Same-day procedures and point-of-care tests occasionally lagged behind E/M capture, creating underbilling risk and denial noise when claims finally dropped.

What we did

  • Defined a same-day charge-capture checklist for visit type, procedures, injections and labs before the encounter closed in the PM system.
  • Separated “verified at check-in” from “eligible for this date of service” when patients returned within the same week.
  • Added a brief coding review lane for laceration, splint and procedure-heavy visits that commonly fail NCCI or documentation edits.
  • Created a daily incomplete-charge work queue with named owners and follow-up timestamps.
  • Reported clean-claim focus areas and aging by payer so leadership could see whether volume growth was producing billable, collectible claims.

Operational outcomes observed

Incomplete same-day charge rates fell after the checklist and ownership model were enforced. Eligibility-related rejections for returning walk-ins decreased when date-of-service verification became routine. Leadership gained a clearer view of which payers drove aging versus which issues were internal capture gaps. Financial results still depend on payer mix, fee schedules and documentation; this summary does not claim a fixed collection lift for all urgent care clinics.
